To optimize your first visit and long-term care, we encourage you to connect with your care team through our secure online patient portal, MyChart (MyPatientChart.org). By completing the necessary forms and providing relevant information prior to your visit, your care team can review your medical history and prepare for a productive and efficient appointment.
One week prior to your appointment, you will receive a notification to e-check in, including a health history questionnaire. We recommend completing this process as soon as possible, or at least four days before your scheduled visit, to maximize the benefits of your appointment. This allows our team to address your concerns more effectively and provide the highest quality of care.
