To help us optimize your first visit and long-term care, we highly encourage you to connect with your care team prior to your first appointment. You can do this easily through our secure online patient portal called MyChart (MyPatientChart.org). By completing the necessary forms and providing relevant information prior to your visit, your care team will be able to review your medical history and prepare for a productive and efficient visit.
One week prior to your appointment, you will receive a notification to e-check in, which includes a health history questionnaire. We strongly recommend completing this process as soon as possible, or at least four days before your scheduled visit to maximize the benefits of your appointment. Our team will be able to address your concerns more effectively and provide you with the highest quality of care.
